Introducing your Primary Care Navigator (PCN)

Your PCN is Margaret Nesbit, a member of the reception team.  Margaret has attended training in this role and has gained knowledge and tools to navigate you to the appropriate service within North Tyneside.

GP practices are very much the centre of the community, and people call or visit with a need for support with their health, but they also contact the surgery to ask for support with other things.

PCN’s have been introduced into practices to help navigate (guide) people to sources of help and support they need to live well. The PCN’s fit in with a wider strategy of Community Navigators and Care Navigators across Adult Social Care, Families and the Voluntary Sector.

PCN’s are here to guide, listen and work with you to navigate to the sources of help and support you need.  It is NOT to fix your problems but to make suggestions, and know who might be able to help you.
